Portsmouth South Coast Primary Care Network (PCN) is a collaboration between two local Practices. We share the same common goal of working together for the greater good of our local community of patients, carers, and families.

We take pride in our proactive approach to provide a strong personalised care approach, providing social prescribing, care coordination, physio, health and wellbeing support both on an individual level and within the local community.

This service really makes a difference to our patients. We hope that our PCN will continue to provide the best personalised health and care services, supporting individuals with complex conditions and empowering our patients to take responsibility for decisions about their health, making informed choices and taking advantage of the local offers of support.
