Portsmouth Southcoast Primary Care Network is a collaboration of the Trafalgar and Craneswater Group Practices, based in the South of Portsmouth.
Our dedicated and caring multi-disciplinary team care for a population of 38,683 patients in Southsea. Working together with community, mental health, social care, pharmacy, hospital, voluntary, community and social enterprise services, our aim is to offer more personalised, coordinated health and social care.
